Job Description

LifeWay Network is a 501(c)3 nonprofit organization that joins the global movement against human trafficking by providing safe housing for women who have been trafficked and offering education about trafficking to the general public. LifeWay Network (LWN) envisions a world in which human trafficking is abolished and every survivor is strong connected and free. LWN is one of only two organizations in the New York Metro area specifically providing safe housing for women survivors of human trafficking and the only organization providing safe housing to survivors of both labor and sex trafficking who are either foreignborn or domestic. What sets LifeWay Networks Safe Housing Program apart is its community living model. It offers a safe home by welcoming each woman into a supportive caring community of permanent residents. This host community helps her recover from her trauma regain her sense of selfworth and enable her to move from isolation towards reclaiming her independence.


LifeWay Network is seeking a compassionate and experiencedHuman Trafficking Care Coordinatorto provide critical traumainformed care and case management to women survivors of human trafficking within LifeWay Networks Safe Housing program. Reporting to the Director of Safe Housing the Care Coordinator will be a key member of the Safe Housing team to create a supportive healingfocused environment that fosters stability and recovery. This fulltime role is ahybridposition based in the Bronx and Queens NY requiring flexibility for afternoon evening and occasional weekend availability to best meet the needs of our residents.


Responsibilities:

  • Facilitate a safe and empowering space for survivors working collaboratively to set and achieve personal goals that promote healing and selfsufficiency.

  • Assist the Director of Safe Housing and Senior Social Worker in the referral and intake process for potential residents ensuring a compassionate and thorough introduction to our program.

  • Welcome new residents and support them in adjusting to the daily routines of community living.

  • Collaborate with external case management providers at partner agencies in case meetings to coordinate health legal and social services for each resident.

  • Provide case management services to residents without case management services (e.g. referrals from law enforcement or selfreferrals) ensuring referrals and access to health legal and essential social service supports.

  • Provide case management aligned with each residents service plan advocating for resources connecting residents to medical and mental health services legal services education and public benefits and facilitating community navigation and social support networks.

  • Build and maintain relationships with key state and communitybased agencies independent service providers educators and program supporters to enhance service delivery and community integration.

  • Offer supportive traumainformed support services to help survivors address complex trauma and achieve personal growth stability and goal fulfillment.

  • Assist in managing emotional and behavioral crises or emergencies providing oncall support to safe house staff residential aides and host community members in the absence of the Safe Housing Director.

  • Assess and mitigate risks by developing reviewing and implementing individualized safety plans for each resident.

  • Collaborate with residents to create strengthsbased goaloriented service plans supporting them in achieving stability and legal status if applicable.

  • Document all residentrelated activities thoroughly maintaining case notes to support data tracking program monitoring and evaluation.

  • Organize and assist with workshops field trips and support groups for residents staff and the host community to encourage skillbuilding selfcare and community engagement.

  • Participate in regular meetings with the Director of Safe Housing offering insights on program improvement and resident wellbeing.

  • Perform additional duties as assigned to support program and organizational objectives.


Qualifications:

  • Minimal qualifications are MSW or MHC. LMSW or LMHC are strongly preferred.

  • Prior experience working with survivors of human trafficking is strongly preferred or a minimum of 3 years experience in traumainformed care in direct service provision.

  • Experience working in traumainformed care case management or social services ideally with survivors of human trafficking or similar vulnerable populations.

  • Knowledge of human trafficking immigration laws social services and resources for survivors of trafficking including those without immigration status is preferred.

  • Experience providing services in an inhome and community living setting.

  • Thorough understanding of confidentiality and traumainformed care.

  • Computer skills and comfort with webbased applications such as Google Workspace.

  • Demonstrate ability to take initiative problem solve work under pressure handle competing priorities and demonstrate adaptability and flexibility.

  • Excellent written and verbal communication and listening skills.

  • Demonstrate success with community relationshipbuilding.

  • Dedication to serving immigrant communities with a sincere commitment to working on behalf of vulnerable and marginalized populations.

  • Flexibility to work afternoons evenings and occasional weekends as needed to provide consistent support and prioritize clients needs.

  • Fluency in English is required and proficiency in Spanish or French is highly preferred.

  • Must be able to travel within New York City between safe houses located in the Bronx and Queens.

  • Valid drivers license preferred.
