Job Description
Summary
Under the direction of the Director of Quality Performance and Patient Safety the Patient Safety Officer role is responsible for establishing and overseeing the enterprisewide patient safety program for the Concord Hospital Health System collaborating with all levels of clinical and administrative staff to create and maintain a culture of safety and excellence in patient care. This position is critical in advancing patient safety as a strategic priority. Additionally this role will have direct oversight of the Quality Performance and Patient Safety (QPPS) nursing team.
Education
Bachelor of Science in Nursing.
Certification Registration and Licensure
Certification required: CPPS or equivalent
Licensure: Valid New Hampshire Board of Nursing Registered Nurse License.
Experience
Minimum of 710 years of experience in healthcare with significant experience in quality improvement and patient safety including leadership roles.
Proven record of accomplishment of developing and implementing successful patient safety programs and initiatives.
Strong analytical problemsolving and communication skills with the ability to engage and influence stakeholders at all levels.
Knowledge of healthcare regulations accreditation standards and best practices in patient safety
Responsibilities
Strategic Leadership Develop and execute the organizations patient safety strategy in alignment with CHHSs mission and vision. Serve as the primary leader for patient safety initiatives ensuring that patient safety is integrated into all aspects of care delivery.
Oversight of Quality Performance and Patient Safety Nurse Team Directly manage the QPPS Nurse Team providing leadership mentoring and support for professional development.
Program Development and Implementation Design implement and evaluate a robust patient safety program policies and protocols based on best practices and in compliance with CMS state and other regulatory requirements. Ensure that patient safety initiatives are evidencebased measurable and sustainable.
Data Analysis and Performance Improvement Oversee the collection analysis and reporting of patient safety data to identify trends inform decisionmaking and guide performance improvement initiatives. Collaborate with clinical teams to develop and monitor key performance indicators (KPIs) related to patient safety outcomes.
Education and Training Develop and deliver comprehensive training programs on patient safety practices for all staff promoting a Just Culture and culture of continuous learning and improvement. Lead initiatives to raise awareness of patient safety issues and engage staff at all levels in safety practices.
Collaboration and Communication Work closely with executive leadership clinical departments and quality improvement committees to ensure alignment and support for patient safety initiatives. Act as a liaison with external regulatory bodies and accreditation organizations ensuring compliance with all patient safety standards and guidelines.
Risk Mitigation Conduct regular safety assessments and audits to identify potential risks and implement strategies to mitigate them. Facilitate root cause analyses for adverse events ensuring that findings are communicated effectively and used to improve safety practices.
