Responsibilities:
Admission/OASISVisits:
o PerformAdmission / Start of Care OASIS Visits throughout an area of theMetro.
ClientEvaluation:
o Evaluateand assess clients for home health services and create their plan of care.
ServiceRecommendation:
o Recommendservices from other disciplines that have not yet been ordered by thephysician.
Collaboration:
o Collaboratewith clients families physicians and other members of the home care team.
Qualifications:
Licensedas a Registered Nurse in the state of Minnesota.
Drivingis required; must have a reliable vehicle with current insurance and a validDrivers License.
PriorHome Health Nursing experience.
Proficiencywith Medicare & OASIS documentation.
Demonstratedability to work autonomously.
Excellentat planning organizing and communicating.
MilitaryOccupational Classification (MOC) Codes:
