Overview

CarePartners has embarked on a new program in partnership with the Ontario Health at Home Central East and we are looking for Community Support Workers (CSWs) to work within the High Intensity Supports at Home (HISH) program. These skilled CSWs will work with patients and their families to support care while patients transition through the healthcare system. The HISH program focuses in WrapAround Care so that all team members (including Nursing Therapy Personal Support Social Work and Community Support Services etc. are working cohesively to meet the needs of the patient. These PSWs will receive additional training and certification in Gentle Persuasive Approach (GPA).

In this exciting staffing model Guaranteed HISH CSWs will have guaranteed & consistent hours of work week over week and have the opportunity to be a respected member of a larger healthcare team striving to provide high quality patient care and positive transition experience for both the patient and their caregivers.

The hours of work will be 7 12hour shifts within a 2week rotation inclusive of weekends:

Twoweek Rotation

Mon/Tues/Sat/Sun

Wed/Thurs/Fri

0hrs

This is an equivalent of seventyseven 77 hours per biweekly pay period!

We are looking for Community Support Workers (CSWs) to help us bring wraparound patientcentered care to patients in Haliburton ON and surrounding areas.

  • A valid drivers license (G or G2 Classification) access to a reliable vehicle and proof of automobile insurance
  • Valid certificate as a Personal Support Worker Internationally Trained Nurse Health Care Aid Developmental Support Worker Home Support Worker or enrollment in a Nursing program
  • To protect our patients we require current vulnerable sector check screening a clear background check

New Ontario PSW grads may be eligible to * receive a $10000 bonus**

* From an approved PSW program in Ontario (on or after April 1 2024

** in exchange for a commitment to work with us position for 12 months payable at 6 months and 12 months must meet HFO eligibility and HFO eligibility status*

and an additional $10000 relocation bonus***

*** when relocating to rural remote or northern area and committing to working with us for 12 months payable at start and 6 months

What The Role Involves

  • Contributing as a valued member of the interdisciplinary care team
  • Assisting with daily living activities such as personal care (rise and retire) assistance with household tasks respite care medication assistance special functions
  • Promoting a safe environment by demonstrating proper body mechanics safe transfer techniques and home safety
  • Providing cognitive social and/or emotional support to the patient
  • Preparing nutritious varied and economical meals and snacks including special diets as determined by the care plan using proper food preparation storage and hygiene practices
  • Providing patient status reports and participate in patient/family care planning meetings

  • Experience working with patients with significant and complex care needs such as dementia and mental health
  • A valid drivers license and vehicle for which you are insured
  • Community Support Worker (e.g. Personal Support Worker Internationally Trained Nurse Health Care Aid Developmental Support Worker Home Support Worker or completion of first year Nursing program with preference given to those with a PSW certificate and/or equivalency
  • To protect our patients we require current vulnerable sector check screening and a clear background check
