About the Role
The Home and Community Care (HACC) Program for Younger People(PYP) provides support for younger Aboriginal people with disabilities whose capacity for independent living and obtaining secure housing is at risk. Services are targeted to younger people with moderate severe or profound disabilities and their unpaid carers.
Key Responsibilities
- Undertake an initial assessment and make appropriate referrals to meet any emergency accommodation and support needs
- Coordinate the client intake process using the Ngwala SHS Assessment Tool assisting clients to make and support their individual case plans to achieve goals (including timeframes exit plans and regular reviews)
- Continually assess and document clients continuing needs in relation to homelessness support services
- Support clients to integrate into social and community networks to increase independent living
- Liaise with the community key stakeholders and various agencies and provide referrals
- Participate on relevant interagency working communities and conduct care plan conferencing when required.
- Ensure client has information on relevant community resources available and assist client to access services and benefits
- Advocate on behalf of clients where appropriate
- Transport clients to various appointments
- Support clients with education employment training finance transport health court mental health issues domestic violence and childcare.
- Maintaining accurate and up to date client records and case notes in the clients file
- Maintain a high standard of professionalism including client confidentiality
- Conduct client feedback and relay such feedback to management
Your Skills & Experience
- A qualification in social work social housing welfare work and / or community development would be highly regarded
- Knowledge and understanding of Aboriginal culture and commitment to the Aboriginal community
- A demonstrated understanding of the issues associated with homelessness and their effects on the Aboriginal Community
- Demonstrated skills in case management including referral pathways and community stakeholder and support services.
- Able to develop positive relationships with clients and work within a stressful environment
- Demonstrate commitment to the care and support of homeless and disadvantaged men women youth and families
- Computer literacy skills including Microsoft Office suite and client management systems
Mandatory Requirements
- Current Victorian Drivers License
- National Police Certificate (no older than 3 months).
- Victorian Employment Working with Children Check
