Description

Community Based Therapist; LSW or LPC; Intensive HomeBased Treatment

Compensation:

Salary: $63000 First year signon and retention bonuses: $11000

Territory: Fayette County with some overlap into surrounding counties

The Intensive HomeBased Treatment (IHBT) Clinician works to make a meaningful difference in the lives of the children and families served. This position works closely with youth family foster parents and community partners providing counseling or therapy advocacy and support toward living safely and successfully in the community.

NYAP is participating agency in the Great Minds Fellowship and Great Minds Workforce Commitment Incentive Programs.

Responsibilities

  • Provides individual group and family mental health therapy case management transportation and other professional services to clients served

  • Managing a small caseload you will travel daily to provide InHome Services to and on behalf of youth and families in compliance with organizational contract and regulatory requirements.

  • You will meet with each client 23 times per week.

  • Coordinate and monitor services for the youth and family in the community mental health medical educational psychological vocational and social services.

  • Regularly reviews and modifies youthbased plans to assure consistent progress and success on written plan goals.

  • Establishes and maintains strong relationship with assigned foster parents and families of origin.

  • Assists in family reunification and/or stabilization efforts with the youths primary family or other permanent living arrangement consistent with the youths case plan.

  • Provides safety assessment crisis response and behavioral stabilization services as required to assure safety and stability for youth served.

Minimum Qualifications

  • Masters degreein Social Work Counseling or comparable Human Services field

  • LSW or LPC required

  • Experience in providing both individual and family therapy

  • Experience working in direct service with youth and families strongly preferred

  • Experience with TFCBT preferred

  • Daily travel required with the willingness to meet clients on a flexible schedule or during nontraditional hours if needed

  • Must be comfortable providing services in the community and in client homes.

  • On Call Duties

  • Productivity: 40 productivity required

  • Proficient use of desktop and laptop computers smart phones and tablets printers fax machines and photocopiers as well as software including word processing spreadsheet and database programs.

  • Must be willing to accept other job duties as assigned

Driving and Vehicle Requirements

  • Valid drivers license
  • Reliable personal transportation
  • Good driving record
  • Minimum automobile insurance coverage of $100000/$300000 bodily injury liability
