Job Description
Clinical Care Coordinator
Community Bridges Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential outpatient inpatient patientcentered medical homes medicationassisted treatment and crisis services to individuals experiencing crisis opioid use disorder homelessness and mental illness. Community Bridges Inc. (CBI) is the premiere nonprofit fully integrated healthcare provider of substance use and behavioral health programs in Arizona including prevention education and treatment using cuttingedge nationally recognized evidencebased models.
Job SummaryA Community Bridges Inc. (CBI) Clinical Coordinator provides clinical and administrative supervision clinical oversight training and mentoring for Case Managers and Rehabilitation Specialist on an assigned team. Ensures clinical quality and productivity standards are met and exceeded by direct reports. Ensures assessments and treatment plans are developed and implemented for each patient on their team.
Skills/Requirements:
Bachelors degree in a field related to behavioral health OR a combination of education training and/or experience
1 year experience working with the SMI population with additional experience in multiple settings and levels of care including both mental health and substance abuse
1 year experience in a behavioral health position providing outreach and engagement activities.
Ability to pass criminal background check for jail visitation/clearance
Valid Arizona Drivers License with 39 month clear MVR
Behavioral Health Technician (BHT)
CPR/First Aid
