Position Details:
Hartford HealthCare is seeking a fulltime experienced Nurse Practitioner or Physician Assistant for our outpatient clinic sites serving both as primary care and as a consultant to our geriatric patients in the greater Farmington area.
Role:Open to Nurse Practitioners (FNP ANP AGNP) and Physician Assistants (PA)
Location: 100 ambulatory based in the Farmington CT area
Setting:Geriatric Medicine in the clinic setting working within a multidisciplinary care team
Schedule:Fulltime or parttime weekdays

Nurse Practitioners and Physician Assistants Role Scope:
Under established guidelines and in collaboration and/or supervision with the practice physician(s) Nurse Practitioners and Physician Assistants perform routine physical examinations and formulate medical educational and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs e.g. referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the providers practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
Requirements:
Licensure: Unrestricted State of Connecticut Medical License (or eligible)
Training: APRN (Family Adult/Geri Nurse Practitioner) or PA
Experience: 1 years experience in a geriatrics or related setting
o New graduates with nursing experience in geriatrics or Skilled Nursing Facilities are welcome to apply!
