Wellvance provides quality prevention education and mental health services in a fiscally responsible manner in Iosco Ogemaw and Oscoda Counties which are aimed at improving the health and welfare of persons served promote greater independence and improve the quality of life for people in these counties with developmental disabilities mental illness or substance use disorders.
Case Manager
The Case Manager is responsible for advocacy coordination and linkage of services to provide continued care and to avoid rigidity fragmentation misuse and/or inaccessibility of services. Must possess the ability to link the consumer to services and coordinate the various services to achieve successful outcomes. Provides services to assist the consumer in gaining access to needed medical social educational therapeutic and other support services. Major functions and core elements of the role includes assessment service plan development linking and coordination of services reassessment/followup and monitoring of services and advocacy.

Candidates must be able to work a flexible schedule and travel throughout the Boards coverage area and at times around different areas of the state. Possession of a drivers license valid in the State of Michigan vehicle insurance as required by Michigan Law and a good driving record; be a U.S. Citizen or have possession of necessary papers allowing holder to work legally within the United States.
