PRN (As Needed)

JOB SUMMARY

Works with patients and family in providing Hospice care in accordance with the plan of treatment approved by the physician. Assigns and supervises Home Health Aides in Hospice care delivery of personal services. Supportive of the Villages mission values and service/hospitality (PRIDE) expectations. Completes training hours as required by policy.

ESSENTIAL JOB FUNCTIONS

  • Performs initial and ongoing nursing assessments develops plan of nursing care in accordance with physicians plan of treatment; provides skilled nursing services in accordance with current nursing practice; evaluates patients response to treatment.
  • Maintains and applies knowledge regarding Medicare and Medicaid regulations.
  • Documents care given and evaluation of treatment in patient records.
  • Maintains medical records in accordance with Agency local state and Federal regulations.
  • Accurately and timely completes required documentation of nursing care to meet all state and Village requirements.
  • Supervises Home Care Aides while providing care to patients on caseload.
  • Works closely with physician Social Worker Chaplain and nursing aides to coordinate appropriate and timely care.
  • Attends and provides input at committee meetings and other scheduled meetings as appropriate.
  • Manages patient caseload and coordinates all services provided.
  • Acts as role model and educator for students in health services areas of study.
  • Maintains competency in nursing practice by attending appropriate educational sessions at a minimum of 15 hours per year.
  • Abides by Agency policies and procedures and interprets them to personnel clients physicians and public.
  • Participates in performance improvement activities.
  • Initiates and participates in case conferences as pertains to patient care to be delivered.
  • Communicates with patients and responsible parties concerning the treatment program charges and insurance program.
  • Gives information and teaches patient families and staff in treatment of patient and maintenance of healthful environment.
  • Applies independent emergency measures to counteract adverse developments and notifies physicians of patients condition.
  • Coordinates Home Care with community agencies furnishing assistance to patients.
  • Adheres to all safety regulations and requirements.
  • Handles stress well and gets along with others within the workplace and with customers outside the department.
  • Maintains acceptable standards of workplace conduct.

The responsibilities described above represent the primary responsibilities of the job. Other responsibilities may be assigned by the supervisor as warranted by business needs. The incumbent is expected to perform all assigned responsibilities.

ESSENTIAL QUALIFICATIONS

Licenses and Certifications:

  • RN current license in State of Missouri required.
  • CPR Certification for healthcare required.

Experience:

  • Oneyear experience in Hospice nursing preferred.

SPECIAL WORKING CONDITIONS

  • All associates may be called upon to assist other departments in a declared emergency situation.

Maximum occasional lift of 50 lbs from floor to waist level; maximum occasional lift of 10 lbs from floor to shoulder; maximum occasional carry of 5 lbs for 100 ft; maximum occasional push/pull horizontal of 40 lbs at 36 in height for 100 ft; maximum frequent push/pull horizontal of 10 lbs at 46 in for 100 ft; maximum occasional grip force of 40 lbs; maximum occasional lateral pinch force of 15 lbs; maximum occasional climb of 8 in; frequent forward reach; frequent bend; frequent fine motor coordination; occasional foot coordination; occasional balance; frequent sitting; continuous standing; and continuous walking. SUPERVISION RECEIVED

Frequently receives supervision from Hospice Supervisor and/or Director of Hospice.

SUPERVISION GIVEN

Home Health Aides assigned to patients on caseload.
