Key Responsibilities:
- Conduct comprehensive patient assessments including physical exams mental health evaluations and diagnostic testing.
- Develop and implement evidencebased treatment plans in collaboration with the healthcare team.
- Provide ongoing patient education support and counseling to promote health and wellbeing.
- Manage chronic conditions and acute illnesses ensuring continuity of care.
- Participate in case management and care coordination to optimize patient outcomes.
Qualifications:
- Current licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in Oklahoma.
- Certification in your specialty Family Practice
- Minimum of 2 years of experience in a clinical setting preferably in integrated or primary care.
- Strong interpersonal and communication skills.
- Commitment to patientcentered evidencebased care.
