Job Description

Health Navigator and Outreach

City: Merritt
Program and Site Location:
Merritt 2196 Quilchena Ave
Reports To: Director of Operations Merritt
Term:
FullTime 37.5 hours per week)
Hours:Monday to Friday 8:30am to 4:30 pm with hour unpaid break
Wage Range: $25.95 to $29.76 based on education and related experience.

Position Summary: As a Health Navigator and Outreach Worker you will work directly with Program Participants and sometimes family members to advocate educate reinforce and coordinate a care plan in order to improve knowledge and assist with selfmanagement of chronic illness and other health issues. To raise awareness and provide sexual health and BBI education/supports and health navigation to marginalized clients and community. Service delivery is based on a client centered nonjudgmental perspective.

Typical Duties Include:

Health Navigator Responsibilities

Liaise with key community caregivers regardingnecessary support systems for the Program Participants including health andlife skills programming.

Engage with community to buildrelationships and provide support.

Provide crisis support and assessment whenrequired.

Assist and support ProgramParticipants in offering information on sexual health HIV and other servicesthat meet the needs of cisandtransgender gay bisexual queer twospirit inMerritt and surrounding communities.

Identify issues in HIV prevention and sexualhealth education.

Raise awareness and provide sexual health andBBI education/supports and health navigation to marginalized ProgramParticipants and community.

Participate in the assessment goal settingand program planning for Program Participants when requested.

Provide supervision and support to mitigate individualand program risk to ensure safety comfort and wellness.

Work closely with identified communityresources for each Program Participant.

Actively seek the views of ProgramParticipants and involve them in decision making to improve their own identitypurpose and selfesteem.

Complete wellness checks/room checks whenrequired.

Monitor and move along community members whoare loitering in and around the property.

Provide crisis intervention as requiredincluding responses by ensuring immediate access to medical services andcompleting necessary incident reports when required.

Delivery of harm reduction supplies tocommunity partners and other members.

Facilitate Naloxone training for Employees ProgramParticipants and community.

General Administrative Responsibilities

Conduct assessments of potential ProgramParticipants using the BC Housing mandated Vulnerability Assessment Tool (VAT)when requested.

Complete accurate and up to datecommunication/case notes.

Maintain files and statistics as per accreditationstandards.

Complete move in/move out documentation asrequired by the Property Services Department.

When requested be a member of the SocietysPerformance and Quality Improvement (PQI) team.

Ensure compliance with all Society policies andprocedures.

Other duties which may or may not be outlined within thisjob description.

Qualifications and Requirements

Bachelor ofSocial Work Human Services Diploma Support Worker/Care Aid Certificate or anequivalent combination of education training and/or lived experience.

Strong ability torecognize and address difficult behaviors.

Goodcommunication documentation interpersonal and conflict management skills.

Ability to workwithin a teambased environment involving multiagency supports.

Strong workingknowledge of community resources.

Understands andmaintains professional boundaries.

Valid driverslicense safe driving record and reliable/appropriate transportation withbusiness use coverage.

VAT(Vulnerability Assessment Tool) training an asset.

Must pass anacceptable criminal record check for the vulnerable sector.

OccupationalFirst Aid Level 1 required.

Readiness tocomplete inhouse online training within the first month of employment.

Mental HealthFirst Aid ASIST and Trauma Informed Practice an asset.
