As a Mental Health Clinician youll work in a collaborative multidisciplinary team ensuring clients receive comprehensive personcentered care that addresses mental health substance use physical health crisis intervention and longterm stability. Schedule: MondayFriday 8:00am4:30pm

Salary starts at: $62000 annually and increases with professional experience and licensure/certifications!

Location: Blackburn Center

MINIMUM QUALIFICATIONS:

  • Must meet qualifications for:
    • QMHPR or QMHP
    • LPCA or LPC
    • LMFTA or LMFT
    • CSWA or LCSW
  • CADCR at minimum certification preferred or the ability to obtain one within 1 year.
  • CADC1 2 or 3 HIGHLY Preferred
  • Experience in mental health and substance use treatment preferred.
  • Has the competencies necessary to:
    • Identify precipitating events
    • Gather histories of mental emotional and physical disabilities alcohol and drug use past mental health services and criminal justice contacts
    • Assess family social and work relationships
    • Conduct a mental status assessment
    • Document a multiaxial DSM diagnosis
    • Write and implement or supervise implementation of a treatment plan
    • Conduct and document a mental health assessment
    • Provide mental health treatment and rehabilitative services within ones scope of practice
  • Must meet CCC privileging requirements as required by FTCA.
  • Must have current CPR certification prior to start.
  • Bilingual (English/Spanish) preferred.
  • Must become a certified Directors Custody designee in Multnomah County within the first 6 months of hire.
  • If using a vehicle for agency use: Must possess a current drivers license access to a vehicle qualify as an Acceptable Driver as designated in Central City Concerns Fleet Safety policy; pass an initial drivers training within 60 days of being an approved driver and continued recertification training; and maintain vehicle insurance coverage of a minimum onehundred thousand / three hundred thousand inpersonal auto liability coverage.
  • Must pass a preemployment drug screen TB Test and background check. This includes clearance by the DHS Background Check Unit.
  • Ability to adhere to Central City Concerns drugfree workplace which encourages a safe healthy and productive work environment and strictly complies with the DrugFree Workplace Act of 1988. An employee shall not in the workplace unlawfully manufacture distribute dispense possess or use a controlled substance or alcohol.
  • Must generally have the ability to walk bend stoop kneel squat twist reach pull and lift heavy objects and climb stairs with reasonable or no accommodation.
  • Must adhere to agencys nondiscrimination policies.
  • Ability to effectively interact with coworkers and clients with diverse ethnic backgrounds religious views political affiliation cultural backgrounds lifestyles and sexual orientations and treat individuals with respect and dignity.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Work as a member of the Blackburn Center multidisciplinary team to deliver comprehensive client centered and trauma informed integrated services.
  • Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following informationsharing guidelines.
  • Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  • Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and culturally specific services as requested.
  • Collect information regarding clients history through a combination of interviewing records review and consultation after obtaining written consent.
  • Conduct and document Mental Health Assessments including mental status exam DSM diagnosis and clinical formulation.
  • Develop care plan with client with assistance & input from all of the clients care team providers
  • Participate in outreach activities necessary to offer services and engage participation in a non coercive manner. Escort clients to meetings and appointments as necessary.
  • Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
  • Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients;
  • Collaborate with Residential facilities Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and nonresidential treatment and service planning.
  • Participate in admission and discharge planning for any client needing hospitalization or subacute level of care. Client contact must be made within one working day of admission.
  • Monitor and assure that the health and safety needs of clients are met.
  • Maintain client records using an electronic medical record. Must be documented as required by program standards and funding requirements including documentation of clinical interactions behavioral health assessments & individual service plans.
  • Participate in regular supervision from the Clinical Supervisor seeking additional supervision whenever necessary. Minimum requirement two hours of documented supervision per month.
  • Attend all mandatory CCC trainings in a timely manner.
  • Conduct educational and therapeutic groups for clients using approved curriculum.
  • Coordinate services with all members of clients support network.
  • Participate in quality assurance and quality improvement activities as directed.
  • Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
  • Assist with the development of an integrated treatment program.
  • Adhere to all state and federal privacy regulations including HIPAA and 42 CFR Part 2 and to CCC policies and agreements regarding confidentiality privacy and security. Support compliance with all privacy and security requirements pursuant to community partners and outside providers patient confidentiality agreements including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments as well as to the program administrator.
  • Perform other duties as assigned.
