PURPOSE:
1. To define responsibilities and accountabilities for a Staff Occupational Therapist.
2. The Occupational Therapist organizes and conducts medically prescribed occupational therapy programs.
3. Occupational therapy goals are to restore and maximize function facilitate activities of daily living after disabilities have occurred to prevent further restrictions of function and to help the patient reach his/her maximum performance level within the limits of his/her capabilities.
4. To ensure therapy services and home health services are provided in compliance with state and federal guidelines
POSITION QUALIFICATIONS:
1. Graduation from an occupational therapy curriculum accredited jointly by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association.
2. Current state licensure and AOTA registration.
3. Two years experience as an Occupational Therapist with experience in home care preferred. Minimum one year in a related setting with experience and interest in gerontology and home care preferred.
4. Knowledge of the healthcare field.
5. Ability to effectively communicate problem solve make decisions and teach.
6. Possess interpersonal skills to work with other team members; demonstrated ethics integrity dependability and reliability.
7. Possess a valid drivers license.
8. Maintains current car insurance.
9. CPR certification (adult / child).
10. Annual TB test and/or chest Xray.
11. Annual health clearance.
ESSENTIAL PHYSICAL AND MENTAL REQUIREMENTS:
Physical: Must be able to drive to patients homes; maneuver around objects; climb stairs or work in relatively small spaces as part of home visit; use assessment skills; support move and lift patients of varying weights; stretch and bend as necessary; and write treatment plans progress notes and records.
Occupational Therapist Sensory: Must be able to hear read write and speak clearly in English to effectively communicate and relate with patients caretakers staff physicians etc. Mental: Must be able to understand supervisors directions and make decisions related to patient care and have clinical knowledge and critical thinking ability to effectively plan and provide patient care consistent with standards and regulations.
SUPERVISORY REPORTING RESPONSIBILITY:Home Health Agency Administrator
PERFORMANCE RESPONSIBILITIES AND STANDARDS:
1. Utilizes appropriate evaluation tools for planning and administration of treatment programs as well as an evaluation of treatment effectiveness and equipment in accordance with MD orders. 2. Considers patients own needs and goals conditions and environment and coordinates with ancillary services when planning treatment goals. 3. Plans treatment programs to restore and enhance activities of daily living. 4. Plans treatment programs to develop or restore function through cognitive/perceptual retraining facilitation/inhibition techniques muscle reeducation developmental motor plan coordination exercises and transfer training with ADLs and joint protection techniques. 5. Plans treatment programs to maximize patient performance and safety through environment adaptation use of appropriate adaptation aides and setting up an ADL home program. 6. Plans treatment programs to maximize pulmonary function through breathing retraining relaxation techniques and breathing coordination with ADLs. Use of energy conservation with work simplification techniques. 7. Makes hospital predischarge visits for the purpose of coordination and continuity of care as requested. 8. Provides timely documentation for assessments daily visits summaries and discharge reports. 9. Records and reports patients reaction to treatment and/or changes in the patients condition to patients physician case manager therapy supervisor and other disciplines involved in the case. 10. Confers on a regular scheduled basis with other team personnel to facilitate coordinated care. Participates in weekly Interdisciplinary Team meetings or provides an oral or written report. 11. Instructs patient family members and caretakers in home programs to be continued in the therapists absence. 12. Instructs and supervises other agency personnel in occupational therapy procedures and techniques which are performed as an adjunct to the occupational therapy plan of treatment. 13. Contacts appropriate health team members when additional modalities and/or services are required and notifies appropriate personnel when services cannot be provided in the home. 14. Assesses need for and provides available resources for outpatient referrals if indicated or ordered by MD. 15. Participates in inservice education programs for agency members and staff. 16. Participates in staff meetings. 500.62 Occupational Therapist 17. Organizes and maintains schedule of assigned patients. Communicates therapy schedule to the patient family and other health team personnel to facilitate coordination of care. 18. Participates in quality improvement activities related to areas of practice as appropriate. 19. Follows documentation guidelines and compliance with state and federal regulations. 20. Fosters interdisciplinary communication and support for optimal patient care coordination. 21. Supervises Occupational Therapy Assistants or students: A. within state regulations B. Be available by telephone during normal business hours. C. Assess the competency of the Occupational Therapy Assistant; determine if their supervision needs exceed the minimum standards D. Be responsible for the overall patient care. Licensed OT Assistants may be supervised offsite; students must be supervised onsite. 22. Participates in the development and implementation of occupational therapy programs to improve quality of patient care. 23. Maintains ongoing rapport with home health agency office staff. 24. Organizes maintains and submits accurate and timely billing information. 25. Assists with additional responsibilities as needed.
SPECIAL CONSIDERATIONS
1. May act as a consultant in specific areas of expertise with supervisors approval. 2. May transport staff with supervisors approval.
