A Residential Program Worker assists those who are returning to the community after mental health hospitalization as well as preventing premature hospitalization. This work supports the consumers efforts to reintegrate into or remain in the community thus fostering success in recovery and in building the confidence and selfreliance needed for full independent living in the community.
This important role supports the consumers efforts consistent with the Individual Service Plan (ISP) in implementing goals plans and objectives. You will train the consumer in home safety protocols share observations with supervisor and MDT members as appropriate and document or chart observations behaviors and consumers progress in a timely manner. Transporting consumers in a county vehicle to appointments may be necessary. This position requires close contact with the consumer in his or her home environment. In this capacity you will work on shifts to assure 24hour availability in a small facility in which a limited number of seriously mentally ill (SMI) consumers reside each in their own apartment.
