Job Description
This position is in one of two Central Inpatient Pharmacies at Mayo Clinic Rochester MN.
Responsibilities include medication management services in coordination with other health care professionals to deliver safe appropriate and costeffective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens medication therapy management medication order verification/entry medication distribution medication therapy monitoring and consultation documentation of medication therapy plans and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocoldriven practice as appropriate. Additionally pharmacists are active members in quality improvement research activities and education.
The night pharmacist team covers all areas of the hospital including intensive care units pediatrics emergency/surgical transplant labor and delivery and many others.
Qualifications
Qualifications
- Graduate of an ACPEaccredited Doctor of Pharmacy (Pharm.D. program or Bachelor of Science (B.S. with a minimum of 2 years relevant experience.
Preferred Qualifications
- Completion of pharmacy residency and specialization (e.g. PGY2 and respective BPS certification) if applicable.
- Evidence of ongoing scholarly pursuits including research/quality improvement publications or scientific presentations.
- Relevant applicable pharmacy experience working in a progressive and collaborative practice environment.
- Experience and/or interest in teaching and/or precepting.
- Commitment to excellence accuracy attention to detail and teamwork required. Ability to communicate effectively work collaboratively with others organizes time well practice with an agile mindset solve problems and work independently with minimal supervision required.
License or Certification
- Current pharmacist license by the MN Board of Pharmacy or eligible to obtain within a time frame compliant with the sites applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
