Job Description
The Emergency Department RN Clinical Case Manager will facilitate transitional care for an assigned number of cases by managing coordinating facilitating and monitoring resources to achieve optimal clinical outcomes utilization of services and financial goals. The Emergency Department RN Clinical Case Manager oversees the discharge process for all patients within the service area and is an integral team member in efforts to facilitate throughput throughout the hospital system.
NYS licensed RN (or eligible) Bachelors degree in Nursingor ahealthcarerelated field plus a minimum of three 3 years acute care RN or acute care transitional care or chronic care RN/case management experience OR Associates degree in Nursingandfive 5 years acute care RN or acute care transitional care or chronic care RN/case management experience required. *If Associate in Nursing must beenrolled in an accredited BSN program with an anticipated graduation date within the next 2 years. Will be appointed temporary until receipt of BSN. CPRBLS certification is required by the time of appointment.
Masters degree in Nursing or healthcarerelated field and prior acute care case management experience knowledge of Interqual Criteria and/or QI/UR experience preferred. Emergency department experience preferred; PRI certification a plus.
MondayFriday with some weekend and holiday coverage.
