An Associate Professional 10A NCAC 27G .0104 must have one of the following:
Bachelor degree (non Human Services) with less than 4 years Full time postgraduate applicable MH/DD/SA experience required
Bachelor degree (Human Services) with less than 2 years Full time postgraduate applicable MH/DD/SA experience required
Master degree (Human Services) with less than 1 year Full time postgraduate applicable MH/DD/SA experience required
1.Assist the Team Leader in gathering relevant facts of psychosocial history.
2.Participate in the personcentered planning process.
3.Assist with implementing a homebased behavioral support plan with the youth and his or her caregivers as indicated in the PersonCentered Plan.
4.Provide psychoeducation as indicated in the PersonCentered Plan.
5.Assist the team leader in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward goals identified in the Person Centered Plan.
6.Assist with crisis interventions. Provide 24/7/365 oncall access to families served by the intensive inhome team and be a first responder to families in cases of emergencies.
7.Assist the team leader in consulting with identified providers engage community and natural supports and include their input in the personcentered planning process. Assist individuals and families in developing natural resources and make contact with social support networks.
8.Assist individuals and families in accessing community resources including locating recreational activities housing food clothing school programs vocational opportunities or services providers to teach life skills and relevant mental health developmental disabilities and substance abuse services.
9.Assist the Team Leader in identifying strengths and utilizing those strengths in order to help individuals and families achieve their stated goals/dreams.
10.Maintain necessary medical records in compliance with state federal and agency guidelines.
11.Attend educational trainings/seminars to remain up to date on current best practices and maintain required certifications and competencies.
12.Preserve positive working relationships within the communities served including individuals families staff monitoring and licensing agencies organizations funding resources and other human services agencies.
13.Drive and travel extensively to community locations various agencies and other outreach destinations. Provide and/or arrange for transportation for individuals and families as required.
14.Participate in the ongoing quality improvement activities of the agency.
15.Follow service definition guidelines for services being provided.
16.Demonstrate knowledge of and comply with all agency policies and procedures.
