Make a Difference
The diabetes educator provides an educational experience for patients and their significant others to promote effective diabetes selfmanagement. The role of the diabetes educator is multidimensional with boundaries for accountability to self patient profession and other members of the health care team and some responsibilities include assesses patient strengths/educational needs using available data and resources; collaborates with team to establish/prioritize individualized plan with patient/significant other; incorporates generally accepted technology and knowledge into patient education experience based on standards of practice for own discipline ADA AADE and other regulatory agencies; promotes safe practice of diabetes selfmanagement related to medication administration meal planning exercise blood glucose monitoring and other aspects of care; empowers patient/significant other with knowledge and skills in selfcare for optimal health and psychosocial adaptation to maintain/improve quality of life; consults with other health care professionals and organizations to utilize appropriate resources (human financial and material) which promote consistency and continuity of care of high value; and other related tasks as needed. The position is located on the north side in Indianapolis Indiana and will be expected to travel to other clinics on the north side weekly.
Exceptional Skills and Qualifications
Graduate of National League for Nursing (NLN) Commission on Collegiate Nursing Education (CCNE) Accreditation Commission for Education in Nursing (ACEN) or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing or three years of related professional nursing experience. Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC) OR Licensed as a Registered Dietician in Indiana (LD) required and Registered Dietician Certification (CDR) required and will consider a Registered Nurse or Registered Dietician.
Bachelors Degree required.
Diabetes educator certification required (CDE CDCES or BCADM) within one year of hire.
Two years or more of Registered Nurse or Registered Dietician experience required.
One to two years of educating patients in the area of diabetes preferred.
Excellent communication skills.
Excellent team skills.
Compassionate and caring.
Accountable and selfdirected.
EPIC experience a plus.
