Job Description
Matrix Providers is hiring a Licensed Clinical Social Worker to join our team of talented professionals who provide health care services to our Military Service Members and their families at Eielson AFB in Fairbanks Alaska.
- Employment Status: Fulltime
- Compensation: This is a salaried position paid biweekly OR This is an hourly position paid biweekly.
- Schedule: Monday to Friday

Requirements: The Social Worker must have and meet the following:
- Degree: Masters degree in Social Work (MSW).
- Education: Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE).
- Certification: Recommend to possess certification as a Certified Advanced Social Work Case Manager (CASWCM) by the National Association of Social Workers but not required.
- Experience: Possess experience of 12 years of Case Management or Clinical experience.
- Licensure: Actively working to obtain current full active and unrestricted license to practice as a Licensed Clinical Social Worker to be completed by 1 year upon hire.
Job Summary:
- Participate in Utilization Management/Case Management meetings to review and identify opportunities to improve treatment provided and recommend corrective action when problems exist.
- Participate in the provision of monthly inservice training for non healthcarepractitioner members of the clinical and administrative staff on subjects germane to social work and social work CM services.
- Primary contact to coordinate admission discharge and transition to and through higher levels of care.
- Focal point for community patient and other agency referrals. Interface with the MTF RN Case Manager(s) in the development and implementation of the Case Management Program (CMP).
- Develop local strategies that use the strength and disciplines of both nursing and social work to accomplish case management.
- Provide assistance in the development and implementation of educational programs.
- Additionally the social work case manager may be required to provide those services identified under Licensed Clinical Social Worker.
