The Contra Costa County Health Services Department is offering an excellent employment opportunity for qualified individuals interested in the Community Health Worker Specialist (CHWS) position. There is Currently one 1 vacancy in Enhanced Care Management (ECM) and three 3 vacancies in Family Maternal and Child Health (FMCH) located in Concord CA. You may be required to occasionally travel to different locations throughout Contra Costa County.


The ECM program provided through MediCal by the Contra Costa Health Plan offers comprehensive support to a specific population of pediatric and adult clients. Within this program our dedicated CHWS staff play a crucial role as inperson case managers working directly with clients in their homes clinics shelters or community settings.


In their capacity as case managers CHWS professionals focus on addressing the diverse care needs of clients including social medical and behavioral health requirements during oneonone inperson field visits. CHWS professionals act as liaisons facilitating communication and collaboration between clients communitybased providers healthcare agencies school districts and social organizations throughout the county.


FMCH programs seek to ensure that all women of childbearing age infants children adolescents and their families have access to quality health services particularly those that reduce the incidence of low birth weight and premature delivery maternal and infant mortality preventable diseases and debilitating conditions.


These Public Health initiatives use public funding and operate under the requirements of the California Department of Public Heath (CDPH) at the local health jurisdiction to provide premiere prevention and intervention services for Contra Costa families in supportive culturally driven strengthbased affirming and innovative ways. We raise awareness of the impacts of the social determinants of health utilize the talents of a multidisciplinary team partner with the community and amplify the individual and systems level strategies to create health equity where we live work and receive healthcare.


FMCH programs include Black Infant Health (BIH) Perinatal Equity Initiative (PEI) Community Oral Health (COHP) Comprehensive Perinatal Services Program (CPSP) NurseFamily Partnership Prenatal Care Guidance (PCG) and Adolescent Family Life Program (AFLP).




We are looking for someone who:

What you will typically be responsible for:

  • Providing lead direction to Community Health Workers which may include the assignment of tasks the distribution of client cases training of workers reviewing their work and providing input on their performance
  • Independently conducting initial client and family needs evaluations in accordance with clearly defined protocols and provides health information and guidance to clients and family members accordingly
  • Interviewing clients and their families in the clinic and/or at home to develop a social/health care needs plan in accordance with protocols or in consultation with the supervisory staff or Public Health Nurse professional staff or nursing staff
  • Providing health information to clients on issues such as infant care and clothing contraceptive methods dental health nutrition prenatal care etc
  • Providing orientation to clients regarding available program services
  • Initiating contact with health and social service providers
  • Maintaining stock of educational materials

Minimum Qualifications

License Required: Possession of a valid California Motor Vehicle Operators License. Out of state valid Motor Vehicle Operators License will be accepted during the application process.

Education: Possession of a high school diploma G.E.D. equivalency or a high school proficiency certificate.

Experience: Three 3 years of fulltime or its equivalent experience having direct responsibility or participation in providing community outreach client education training and support services in a community program that promotes health and health education.

Substitution for experience: Completion of 60 semester or 90 quarter units of college coursework must have included 6 semester or 9 quarter units in health related coursework or completion of a Community Health Worker Associates Degree or Community Health Worker Certification Program from an accredited college or university may be substituted for twelve 12 months of the required experience.


Desirable Qualifications:

  • Ability to understand and speak Spanish

Selection Process

  1. Application Filing and Evaluation: All applicants will be required to complete a supplemental questionnaire at the time of application. Applications will be evaluated to determine which candidates will move forward in the next phase of the recruitment process.
  2. Oral Interview: Candidates who possess the minimum qualifications will be invited to participate in an online assessment. The online assessment will measure candidates competencies as they relate to the job and candidates must achieve an average passing score of 70 or higher on each of the competencies as well as an overall passing score of 70 or higher. The Oral Interview is tentatively scheduled to take place via computer (remotely) during the week of April 23 2025.


The Oral Interview will be administered remotely using a computer or mobile device such as a tablet or smartphone. You will need access to a reliable internet connection to take the assessment as well as a computer or mobile device with a camera.
