GENERAL DESCRIPTION:
The employee will provide recoveryoriented services as outlined by the Texas Administrative Code and in accordance with the Texas Health and Human Service Commission Resiliency and Recovery Management principles. Specifically the employee will provide Mental Health Case Management functions as outlined by Texas Administrative Code Title 25 Part II Service Coordination Chapter 412 Subchapter I to a designated caseload of TCOOMMI individuals that have been referred from local Adult Probation & Parole Department. Services include assessment treatment planning monitoring of services skills training case management and crisis prevention and management. Employee will develop and maintain referral resources for service provision allowing for consumer choice and comply with Texas Correctional Office on Offenders with Mental/Medical Impairments (TCOOMMI) Program Guidelines. Employee will ensure best value of resource allocation to service provision. The focus of services is to stabilize symptoms reduce the risk of recidivism increase awareness of and participation with community and natural supports develop skills in selfadvocacy and gain ability to participate in independent mental health care. Also this employee will be cross trained with other TCOOMMI Programs with MHMR Services for the Concho Valley to provide TCOOMMI coverage as needed. The employee will implement the principles of traumainformed care including culture sensitivity in all areas of service delivery.
