Under the direct supervision of a pharmacist performs transition of care (TOC) related tasks such as collection and verification of allergy and medication history. The TOC liaison will communicate with patients families nurses physicians and other healthcare professionals to obtain accurate home medication lists upon hospital admission. Provides integrated support for patients and the hospitals admitting team to ensure a safe medication transition by providing a handson approach to total quality patient care.
Major Responsibilities:
Provides integrated support to patients pharmacists and various healthcare providers by providing a handson approach to total quality patient care.
Works closely with patients and/or their families to obtain the best possible medication and allergy history. Includes:
Interviewing patients and/or their families
Utilizing active listening skills
Utilizing available resources to overcome barriers (i.e. language barriers)
Understanding basic medication information
Integrate and collaborate with the emergency departments admitting clinical teams consisting of physicians nurse practitioners nurses and case managers (i.e. Transition of Care Team ED Team Medicine Team Surgery Teametc..
Operate electronic health records to obtain patient medication history information.
Contact outside medical facilities to obtain patient medication history information. Includes:
Pharmacies
Physician offices
Hospitals
Longterm Care Facilities
Position Qualifications:
License/Certification/Education:
Required:
High school diploma or equivalent required.
Massachusetts pharmacy technician registration and national pharmacy certification by Pharmacy Technician Certification Board (PTCB) required.
Experience/Skills:
Required:
Three years pharmacy experience required with both community and inpatient hospital experience preferred.
Must be proficient in math with knowledge and ability to use a computer for data entry.Must be able to follow oral and written instructions with accuracy efficiency and dependability.
