The Field Care Manager Behavioral Health 2 assesses and evaluates members needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

The Field Care Manager Behavioral Health (Care Manager Behavioral Health) employs a variety of strategies approaches and techniques to manage a members physical environmental and psychosocial health issues. Identifies and resolves barriers that hinder effective care.

  • Performs telephonic and face to face assessments and evaluations of the members needs and requirements to achieve and/or maintain an optimal wellness state by guiding members/families toward the appropriate resources for the care and overall wellbeing of the member.
  • Ensures member is progressing towards desired outcomes by continuously monitoring care through assessments and/or evaluations.
  • Creates member care plans. Understands department segment and organizational strategy and operating objectives including their linkages to related areas.
  • Collaborates with providers and community services to promote quality and costeffective outcomes.
  • Ensures the delivery of needed services/supports for Behavioral Health Physical Health Social Determinant of Health and valueadded benefits.
  • Coordinates across the transdisciplinary care team (at a minimum the PCP) and transitions of care
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • Submits incident reports
  • This role supports the Baton Rogue LA area.

Required Qualifications

  • Masters degree in a behavioralhealth field such as social work counseling or related health discipline from an accredited university and licensed to independently practice mental health in the state of Louisiana

Field Care Manager Behavioral Health must have one of the following:

  • Licensed Clinical Social Worker
  • Licensed Professional Counselor
  • Licensed Addiction Counselor
  • Licensed Marriage and Family Therapist

Field Care Manager Behavioral Health must meet all the following:

  • 2 years or more of experience working as a behavioral health professional
  • Experience with behavioral change health promotion coaching and/or wellness
  • Proficiency with Microsoft Office Programs e.g. Word Excel & PowerPoint shared systems troubleshoot and resolve general technical issues
  • Must reside and perform work in the state of Louisiana

Preferred Qualifications

  • Experience supporting patients telephonically
  • Experience working in Community Mental Health or as part of a crisis response team or Assertive Community Treatment (ACT) team and substance use disorder treatment
  • Experience working with both children and adults
  • Bilingual (English/Spanish) Bilingual Language in both English and Spanish Language Proficiency Assessment will be performed to test fluency in reading writing and speaking in both languages.

Travel: Could be up to 75 in the state of Louisiana

  • This position will require a minimum of 75 travel in the State of Louisiana in Region 2 which is the assigned location. However at times coverage may be needed in other Regions due to the needs of the business.
  • Must have a Valid drivers license with reliable transportation and the ability to travel within the state as required
  • This role is part of Humanas Driver safety program and therefore requires an individual to have a valid state drivers license and proof of personal vehicle liability insurance with at least 100000/300000/100000 limits.

Work At Home Requirements

To ensure Home or Hybrid Home/Office employees ability to work effectively the selfprovided internet service of Home or Hybrid Home/Office employees must meet the following criteria:

  • At minimum a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless wired cable or DSL connection is required.
  • Satellite cellular and microwave connection can be used only if approved by leadership.
  • Employees who live and work from Home in the state of California Illinois Montana or South Dakota will be provided a biweekly payment for their internet expense.
  • Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time 40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills knowledge experience education certifications etc.

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
