Physical Therapist Responsibilities:
- Conducting initial and ongoing evaluations of residents physical capabilities including strength mobility and endurance.
- Identifying barriers to independence and functional movement.
- Developing individualized treatment plans to enhance residents mobility strength and functional independence.
- Addressing challenges related to mobility balance and pain management.
- Assisting in recovery from injuries surgeries or illnesses through targeted physical therapy exercises and techniques.
- Helping residents improve their ability to perform activities of daily living (ADLs) such as walking dressing and transferring.
- Implementing strengthening flexibility and coordination exercises to improve motor function and prevent further injury.
- Educating residents and staff on fall prevention techniques and mobility strategies.
- Recommending and training in the use of assistive devices like walkers wheelchairs or braces.
- Collaborating with a multidisciplinary team including nurses occupational therapists speech therapists and other healthcare professionals to ensure holistic care.
- Participating in care planning meetings to set and review therapeutic goals.
- Maintaining accurate records of assessments treatments and progress to ensure compliance with healthcare regulations.

Qualifications
- Graduate of an accredited school with a bachelors masters or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
