Job Description

We currently have a position available for an experiencedHome Health SpeechLanguage Pathologist (SLP) to join our dynamic team!

Responsibilities:

  • Provides Speechtherapy services to patients according to a written physicians plan of care.This may include but will not be limited to: Assessing and evaluating rehabilitative/functional status and participation in the development of the total plan of care. Evaluates home environment.
  • Provides a full range SpeechLanguagePathology Services as ordered by a physician.
  • Improves or maximizes the communication of the patient.
  • Assist/develop the plan of care and review as necessary.
  • Prepares and submits clinical and progress summaries based on the attainment of goals.
  • Participates in discharge planning for patients.
  • Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.
  • When therapy is the only skilled service instructs supervises and evaluates home health aide care every two 2 weeks.

What we are looking for:

  • Masters degree from aSpeechLanguagePathology program certified by the American Speech Association to practice in the state of Michigan
  • License and certification as a SpeechLanguagePathology issued by the Michigan state
  • Minimum of oneyear healthcare experience
