Summary
Job Summary
In collaboration with a physician diagnoses and treats certain acute or chronic health conditions in specific populations.
Job Responsibilities
- Obtains patient history and performs assessment via observation interview and examination.
- Orders performs and interprets diagnostic studies.
- May be required to perform preventative health assessments screenings immunizations and care.
- Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources.
- Provides counseling and education of patients and their families/caregivers concerning preventative health treatment options and community resources.
- Provides care in the context of promoting age and cultureappropriate informed choices shared decision making and the right to selfdetermination.
- Provides accurate and timely documentation of assessment and patient care plan.
- May coordinate the registration functions including collecting and entering demographic insurance employer information and copayments.
Qualifications
- Masters degree or higher in Nursing required upon hire
- No experience required
- RN Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required upon hire AND
- APRN Licensed as an Advanced Practice Registered Nurse in the state of Kentucky required upon hire AND
- APRN National certification from American Academy of Nurse Practitioners (AANP) required upon hire OR
- APRN National certification from American Association of Critical Care Nurses (AACCN) required upon hire OR
- APRN National certification from National Certification for the Obstetric Gynecologic and Neonatal Nursing Specialties (NCCCOGN) required upon hire OR
- APRN National certification from Pediatric Nursing Certification Board (PNCB) required upon hire OR
- APRN National certification from American Nurses Credentialing Center (ANCC) required upon hire OR
- APRN National certification as Neonatal Nurse Practitioner from National Certification Corporation (NCC) required upon hire
