Work Scheduled: Casual MF with occasional weekend
Required Qualification:
- Graduate of accredited physical therapy program
- Continuing education as required by state licensing boards
- MN or WI Physical Therapy (PT) License upon start date
- Must provide proof of MN and WI PT license within 4 months of start date if both MN and WI PT licenses are not present on start date
- Must have a vehicle with auto insurance and a valid drivers license and meet all criteria required under Lakeviews Automobile Liability Guidelines
- Current BLS American Heart Association or BLS American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
Preferred Qualification:
- Proof of application for alternate state within 4 weeks of start if both MN and WI PT licenses are not present on start date
