GENERAL DESCRIPTION A Geriatric Support Service Coordinator conducts comprehensive inhome assessments with elders helps determine needs and eligibility for governmentsubsidized services creates care plans and coordinates services. Duties include data entry into a computer database communication and consultation with family members and other Aging Services of North Central Massachusetts staff and community professionals.
ROLE AND RESPONSIBILITIES
- Completing face to face memberoriented needs assessments within 30 days of the referral that identify issues or problems which inhibit independent living
- Completing reassessments as scheduled according to the Senior Care Options (SCO) plan contract and level of member
- Authorizing appropriate services under the individual SCO policy and procedures; monitoring care plans to ensure needs are being addressed and services are being delivered as authorized.
- Participating in the SCO interdisciplinary team by making joint member visits with the SCO RN attending Primary Care Team (PCT) Meetings as requested attending monthly or quarterly meetings with the SCO Team and attending SCO training as requested.
- Advocating effectively on behalf of members to assist them in availing themselves of the benefits and programs available via the SCO
- Assisting in problem resolution
- Maintaining a thorough knowledge base regarding community resources and benefits provided by the SCO
- Maintaining case documentation in a timely and comprehensive manner in accordance with the individual SCO contract and standards
- Keeping SCO team apprised of any changes with members
- The GSSC will perform other duties as assigned
QUALIFICATIONS AND EDUCATION REQUIREMENTS
- A Bachelors degree in Social Work or a related field with two years experience in casework or related activity. Experience in elder care or the human service field is preferred.
- The case manager must be able to work with a diverse population make mature judgements possess good communication skills and maintain effective interpersonal relationships. Knowledge of basic medical mental health social service delivery systems as well as acceptable record keeping practices is essential.
- A valid drivers license and a legally insured and registered motor vehicle are required. (A copy of current license and registration will be on file in the personnel record.
PHYSICAL:
- Visual speaking auditory and mobile capacity necessary:
- Capacity to see computer screen read written material and drive a car.
- Capacity to hear and speak on the telephone.
- Capacity to communicate verbally with consumers caregivers supervisors and managers.
- Capacity for fine manipulation in the frequent use of office equipment such as computers copy machines fax machines telephones calculators etc.
- Capacity to drive in allweather to visit consumers in their homes
- Capacity to navigate uneven terrain in allweather to visit consumers in their homes
- Capacity to climb stairs
- Ability to sit or stand for extended periods of time.
- Occasional reaching and grabbing objects with both hands twisting of hand and wrist and pushing and pulling of objects.
- Occasional bending squatting and twisting to perform work functions.
- Occasional capacity to lift up to 25 pounds.
MENTAL:
- Ability to understand and/or interpret complicated program instructions and laws.
- Capacity to learn complicated computer software
- Capacity to deal rationally and calmly with varying personalities
- Capacity to work well in fast paced rapidly changing environment.
ENVIRONMENTAL:
- Work is split between indoor remote office work and traveling outdoors to meetings trainings and consumers homes.
- Must be able to tolerate odors and pollutants including but not limited to smoke air fresheners pet dander personal odors etc. from a variety of inhome conditions.
- Must be able to tolerate byproducts of office machine operation.
- Must be able to tolerate heat and cold of seasonal changes and indoor temperatures.
