Job Description
Ovation Primary Care continues to grow and we are now expanding into the Utah market! We seek a highly skilled and compassionate primary care provider/nurse practitioner focusing on geriatrics hospice and palliative care to join our primary care team. As a Primary Care Provider / Nurse Practitioner with Ovation you will be vital in delivering comprehensive healthcare services to our elderly population.

Responsibilities:
- Provide highquality primary care services to geriatric patients focusing on promoting healthy aging and managing chronic conditions.
- Conduct thorough assessments perform physical examinations and order and interpret diagnostic tests.
- Develop and implement individualized care plans based on patient needs including medication management preventive screenings and health promotion activities.
- Collaborate with interdisciplinary teams including physicians social workers and other healthcare professionals to ensure holistic patientcentered care.
- Assist in coordinating and providing hospice and palliative care services including pain management symptom control and emotional support for patients and their families.
- Educate patients and their families about disease management treatment options and endoflife care decisions.
Requirements:
- Current and unrestricted Nurse Practitioner license in Utah.
- Minimum of 2 years of experience as a Nurse Practitioner focusing on geriatrics hospice or palliative care.
- Strong knowledge of geriatric healthcare chronic disease management and endoflife care principles.
- Excellent clinical assessment and diagnostic skills.
- Ability to work collaboratively in an interdisciplinary team environment.
- Exceptional communication and interpersonal skills with a compassionate and empathetic approach to patient care.
- Ability to travel in the Utah Area.
