Licensed Practical Nurse (LPN) Primary Care and Chronic Care Support

Responsibilities
- Monday Friday 8:00 AM 5:00 PM
- Flexible to occasionally help with later hours on Tuesdays & Thursday (Schedule will be flexed)
- 100 Outpatient Patients of all ages. Volume varies depending on the day and the providers that are in house.
- LPN is not doing case management rather continuity of care.
- Primary Care Duties:
- Provide direct patient care in accordance with the scope of practice including taking vitals administering medications wound care and assisting with minor procedures.
- Document patient encounters maintain uptodate patient charts and assist with patient intake and followup as needed.
- HighRisk and Chronic Care Management:
- Identify and manage patients at high risk for complications including those with chronic conditions such as HIV diabetes hypertension heart disease and COPD.
- Collaborate with providers care managers and other clinical staff to develop individualized care plans that address patients medical and social needs.
- Reports To: Health Centers Manager and/or Clinical Supervisor.
Requirements
- Clear and active LPN in the state of Connecticut
- Knowledge of population health care coordination and referral processes.
- Demonstrated proficiency with electronic health records (EHR) preferably Epic
- Bilingual Haitian Creole preferred
- Preferred minimum of 2 years of experience in a primary care or outpatient setting
- Preferred experience in highrisk or chronic care management
