Job: RN Oncology Nurse Navigator Job Category: 2 Professionals
Position Summary The Nurse Navigator for Oncology Services will be a part of the multidisciplinary team serve as an advocate for the cancer patients be the single point of contact for all the patients providers and ensure patients stay compliant with their plan of care.
Essential Functions Serves as an educator for the patients and provides emotional support Identifies new and established patients with a cancer diagnosis that requirenavigator services Serves as a liaison between the patient the patients family and all providersinvolved in their care Assists patients by obtaining referrals and appointments for social workerdietitian genetic counselor nurse practitioner community or outreach coordinator any clinic consultations as well as support services inclusive of themedical care team Follows patients through the care continuum including inpatient admissions andworks with inpatient care management resources to arrange for any test orappointments following discharge Assists patients to make informed decisions Conducts initial and ongoing comprehensive needs assessments of patients andfamilies Models patient experience guidelines (Compassion Empathy Friendly Warm Responsive Professional Organized Genuine Trustworthy and Positive) in all interactions with the patients Maintains knowledge of professional and industry trends Responds to inhospital consultations as requested Participates in community activities and health fairs Adheres to all Titus Regional Medical Center (TRMC) and The Clinics of Titus(TCOT) policies Reviews Epic Oncology Metrics dashboard along with any other navigator toolsdaily Adheres to established productivity benchmarks quality metrics and clinicaloutcomes and submits data for the oncology dashboard Develops patient education programs and written communication materials Participates in any oncology service line performance improvement activities Performs other duties as requested Follows and adheres to TRMC vaccine policy(s) mandated by the Centers for Medicare & Medicaid Services (CMS).
Work Experience Three 3 years oncology nursing experience preferred. Experience in departments of the hospital required. EPIC experience preferred. Experience with accreditation highly desirable.
Educationand Qualifications Registered Nurse currently licensed to practice in the State of Texas required. Masters degree in nursing (MSN) or masters degree in nursingrelated field preferred. Oncology certified nurse certification required within 6 months of start date Oncology Nursing Society Chemotherapy Immunotherapy Provider card required within 2 years of start date. BLS (Basic Life Support) required upon hire date. Proven success working with all levels of management. Clinical assessment skills. Strong written and verbal communication skills. Strong presentations skills. Leadership style that builds and maintains a climate of trust and inspires others toachieve organizational goals. Active listening skills. Conflict resolution skills. Expert knowledge in medical surgical and radiation oncology. Knowledge of legal and professional guidelines and scope of practice.
