Qualifications for

• Current Montana Licensed Practical Nurse LPN license .

• Current BLS certification required as applicable to assigned area(s).

• Previous experience in home care or hospice preferred.

• Must have capability to connect to internet for daily data exchange.

• To be eligible for the Driver portion of the position Current Drivers License required.

• Driving record for the previous thirtysix 36 months must not reflect any major driving offenses i.e. DUI convicted of a felony involving a motor vehicle suspended license three 3 or more speeding tickets or other moving violations etc..

• In addition personal vehicles used for LH business:State law minimum liability coverage limits required.

• Excellent organizational skills detailoriented a selfstarter possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

• Commitment to working in a team environment and maintaining confidentiality as needed.

• Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

• Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact professionalism and diplomacy.

• Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

• Provides skilled nursing care to patients utilizing the nursing process as appropriate for LPNs in home care/hospice

• Exhibits a commitment to professionalism civility teamwork and customer service while undertaking all tasks and interactions with others.

• Exhibits knowledge of policies and procedures as they relate to the specific job duties patient care and safety.

• Exhibits knowledge of staffing standards and a demonstrated commitment to cost containment with all resources.

• Operates and maintains equipment to departmental standards.

• Maintains nursing standards of care and executes appropriate provider orders. While executing patient care exhibits knowledge of and maintains the patients right to diversity privacy and confidentiality.

• Performs a standardized focused assessment or reassessment of patient at regular intervals appropriate for the condition of the patient under the direction of the RN and as ordered by the provider.

• Administers medications treatment and providerspecific care to patients in compliance with nursing policies/procedures and LPN scope of practice.

• Communicates all appropriate and critical changes in the patients condition status and findings to the appropriate stakeholders in a timely manner and as defined by policies and procedures and/or provider orders.

• Documents all clinical findings and regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements.

• Contributes to maintaining an updated individualized plan of care that includes goals interventions medications and visit frequency orders.

• Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.
