Together We CAN! The Adult Day Care Nurse Supervisormust be licensed as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of North Carolina. We are looking for someone with a compassionate nature knowledgeable about health care practices and provide exceptional customer service to assist patients in handling their illness. - This Nurse Supervisor role is a fulltime day shift position MondayFriday. No night shift or weekend coverage required
Job Responsibilities: - Conduct preadmission health assessments and comprehensive assessments
- Create and implement health care components of service plan including but not limited to medication administration vital sign monitoring bowel/bladder plan
- Conduct biannual care plan meetings for all participants
- Maintain quarterly progress notes for all participants
- Monitor participants response to medical treatment
- Collaborate with other health care providers in regards to participants service plan and all aspects of care
- Educate staff regarding health concerns and service provision for participants
- Provide monthly health care in service training for staff
- Provide first aid treatment to participants as needed
- Supervise all health related personal care provision
- Assist participants with activities of daily living as part of their care plan
- Communicate with families and caregivers regarding any changes in participants health status
- Provide monthly health education for participants and families/caregivers
- Administer medications
- Conduct phone screenings and tours for potential participants
- Assist Director with monthly DAAS surveys
Qualifications: - Must be licensed as a Registered Nurse in the State of North Carolina
- Minimum of one year of related experience in caring for frail or impaired adults
- Current CPR/First Aid/AED certification
- Proficient in computers Microsoft Word/Excel EMR system
- Have a knowledge and understanding of the physical and emotional aspects of aging the resultant diseases and infirmities and related medications and rehabilitative measures
- Prior Adult Day experience preferred
- Prior supervision experience preferred
Other Requirements: - Evidence of negative TB test in the 12 months prior to hiring
- Adult Day Medical Statement from physician required prior to employment
- Must provide 3 references
