Description
Coordinates the clinical teaching/learning aspect of the diabetes program for Diabetes Healthways (DH).
Essential Functions
- Effectively facilitates/coordinates the provision of DH services for assigned patients.
- Facilitates learning by patients families and significant others regarding diabetes management.
- Serves as a resource for the DH team hospital and community regarding DM management.
- Demonstrates self direction in fostering personal expertise and future professional development.
Education Training and Experience
BS degree in Nursing orB.S. Degree in Dietetics or Food & Nutrition
Two years of clinical experience
