Overview: We are seeking an experienced and dedicated Admisson Nurse (RN) to join our home health care team. As an integral member of our interdisciplinary team you will provide skilled nursing care to patients in their homes promoting their independence comfort and overall wellbeing. Your role will involve conducting comprehensive assessments developing individualized care plans and coordinating care with other healthcare professionals to meet patients needs. Responsibilities:

  1. Perform comprehensive nursing assessments of patients in their homes including physical psychosocial and environmental factors to identify needs strengths and goals for care.
  2. Perform comprehensive initial assessments and hand off to the LPN. Completing other Oasis visits as needed for Re certification or Discharge.
  3. Develop individualized care plans in collaboration with patients families physicians and other members of the healthcare team incorporating evidencebased practices and patient preferences.
  4. Provide skilled nursing care and interventions as prescribed by physicians or advanced practice providers such as wound care medication management infusion therapy catheter care and palliative care.
  5. Monitor patients health status symptoms and response to treatment and adjust care plans accordingly communicating any changes or concerns to the interdisciplinary team.
  6. Educate patients and caregivers on disease management medication adherence selfcare techniques and preventive measures to promote optimal health and independence.
  7. Coordinate care with other healthcare professionals including therapists social workers home health aides and community resources to ensure holistic and coordinated care delivery.
  8. Document nursing assessments interventions and patient responses accurately and in accordance with regulatory standards and organizational policies utilizing electronic health record (EHR) systems.
  9. Provide emotional support counseling and advocacy to patients and families coping with illness disability or endoflife issues demonstrating empathy compassion and cultural sensitivity.
  10. Implement infection control practices safety measures and emergency protocols to protect patients caregivers and oneself from harm adhering to established guidelines and protocols.
  11. Participate in interdisciplinary team meetings case conferences and quality improvement initiatives to review patient progress identify barriers to care and enhance care coordination and outcomes.

Qualifications:

  1. Graduation from an accredited nursing program with current licensure as a Registered Nurse (RN) in the state of employment.
  2. Minimum of 23 years of nursing experience preferably in home health community health or a related healthcare setting demonstrating proficiency in skilled nursing procedures and clinical assessments. Experience as a LPN will be considered.
  3. Strong clinical assessment skills critical thinking abilities and proficiency in administering medications treatments and procedures safely and effectively.
  4. Excellent communication interpersonal and patient education skills with the ability to establish rapport with patients and families from diverse backgrounds and cultures.
  5. Knowledge of nursing principles practices and standards of care relevant to home health nursing including regulatory requirements and best practices in communitybased care.
  6. Ability to work autonomously manage time effectively and prioritize tasks in a dynamic and fastpaced home health environment.
  7. Proficiency in using electronic health record (EHR) systems and other technology platforms for documentation communication and care coordination.
  8. Experience with wound management and diagnosis preferred
  9. Valid drivers license reliable transportation and willingness to travel to patients homes within the designated service area.
  10. CPR certification and other relevant certifications may be required depending on state regulations and organizational policies.
