Job Description
Description of the role:
University Home Care is currently seeking a fulltime Physical Therapist to join our team. As a Physical Therapist in home care you will be responsible for providing highquality therapy services to patients in the comfort of their own homes. You will assess the patients needs develop customized treatment plans and help patients improve their mobility alleviate pain and regain independence.
Responsibilities:
- Conduct thorough assessments of patients medical history and current conditions
- Develop and implement individualized treatment plans
- Perform therapy sessions in patients homes ensuring comfort and safety
- Collaborate with other healthcare professionals to optimize patient care
- Educate patients and their families on exercises and techniques for ongoing rehabilitation
- Maintain accurate and detailed patient records
Requirements:
- Valid license to practice as a Physical Therapist in the state of Michigan
- Proven experience in providing physical therapy services preferably in a home care setting
- Excellent knowledge of physical therapy principles and techniques
- Strong interpersonal and communication skills
- Ability to work independently and prioritize tasks
- Flexibility to travel to patients homes within Livonia Michigan
