Job Description

This is a nonrepresented fulltime role that works for 10months a year (with the summers off). The salary range for this position is $24514 $36771 with the final salary determined by our Classification and Compensation team.

Job Summary

Provides assistance to support families and students engaged with the school the Support Team for Educational Partnership (STEP) and outside resources in compliance with Medicaid regulations. Works within the schools MultiTiered System of Support framework to assist parents in addressing their childs social emotional and social service needs. Assists the team in providing support for unmet physical living situation and behavioral health needs to help stabilize the child and family. Serves as a link between the home school and community. This position is developed in collaboration with Community Behavioral Health and the City of Philadelphia.

Essential Functions

As directed by the STEP Clinical Social Work Coordinator and/or the Principal:

  • Provides support to families as part of the development and implementation of a plan with STEP; works to support the needs of youth and families; helps navigate involvement in social support systems and treatment opportunities.
  • Builds trust with families involved with STEP and works closely with other professionals on STEP; serves as a visible resource for students and families.
  • Maintains documentation of all interactions in compliance with STEP Medicaid and School District of Philadelphia guidelines.
  • Interacts with external organizations such as Community Umbrella Agencies the Philadelphia Department of Human Services and treatment providers to support families in being linked in meaningful ways to outside providers and supports.
  • Provides information about resources and supports to students and their families; assists students and their families with identifying and developing support networks.
  • Meet families in the home or at outside agencies to support family engagement.
  • Assists with the collection of data.
  • Participates in in all STEP training opportunities.

Knowledge Skills and Abilities

Demonstrated knowledge of:

  • community organizations providing services to families and children in Philadelphia.

  • personal computer applications such as Microsoft Office Suite that are used to create reports and documents.
  • procedures to protect confidential information such as school and mental health records.
  • the processes for making the final decisions as a parent or primary caregiver for a minor who has been diagnosed with a mental emotional or behavioral disorder.

Demonstrated ability to:

  • work effectively with parents/guardians and students to address their specific concerns.
    • maintain confidentiality and protect sensitive information such as school and mental health records.
  • work independently and collaborate with team members.
  • work in a fastpaced environment and provide support to many families.
  • use a personal computer.
  • communicate effectively both orally and in writing.
  • establish and maintain effective working relationships.

Minimum Requirements

  • High school diploma or equivalent.
  • One year of experience as a parent or primary caregiver with responsibility for making the final decisions for a child or youth who has been diagnosed with a mental emotional or behavioral disorder.
  • All work experience related to the position must be included on the resume.
