Central Community Health Centre
Seeking a Nurse Practitioner eager to launch a mobile adventure! We are a Community Health Centre with our main office in St. Thomas Ontario however we want to get our mobile unit on the road to serve our community better. This position would be ideal for a Nurse Practitioner who loves independence meeting people where they are at and building innovative care spaces. Our area is primarily rural and that is the population we are seeking to serve in a new and exciting way. While you may be on the road much of your week you also have the full support of a multidisciplinary team with Central CHC. If this catches that pioneering bug in you please let us know!
Key Responsibilities:
Deliver highquality patient care in a mobile setting including assessments diagnoses and treatment plans.
Perform routine checkups health screenings and followup visits.
Collaborate with a multidisciplinary team to ensure comprehensive care.
Educate patients on health management wellness and preventive care.
Maintain accurate patient records and documentation in compliance with healthcare regulations.
Participate in community outreach programs to promote health awareness
Qualifications:
Registered in the Extended Class with the College of Nurses of Ontario.
Has minimum three years experience in a community health setting or a combination of community and hospital or public health settings
Has experience in program development implementation monitoring and evaluation
Proficient in the use of computers and various software applications including software used by the Ministry of Health and LongTerm Care and other office equipment.
Confident working in a fastpaced environment and can respond in a positive manner to demanding issues while projecting a welcoming friendly personality.
Low German speaker an asset
Experience in mental health an asset
Must have a valid Ontario Drivers Licence.

$122178 per year
