Responsibilities:
- Conduct comprehensive nursing assessments of patients in their homes including physical psychosocial and environmental factors to develop individualized care plans.
- Perform skilled nursing procedures and treatments as prescribed by the physician or advanced practice provider such as wound care medication administration injections catheter care and monitoring vital signs.
- Monitor patients health status symptoms and response to treatment and communicate any changes or concerns to the supervising registered nurse or healthcare provider.
- Educate patients and caregivers on disease management medication adherence selfcare techniques and preventive measures to promote optimal health and independence.
- Provide emotional support counseling and reassurance to patients and families coping with illness disability or endoflife issues demonstrating empathy compassion and cultural sensitivity.
- Coordinate care with other members of the healthcare team including physicians therapists social workers and home health aides to ensure holistic and coordinated care delivery.
- Document nursing assessments interventions and patient responses accurately and in accordance with regulatory standards and organizational policies utilizing electronic health record (EHR) systems.
- Collaborate with patients families and caregivers to develop realistic goals and discharge plans facilitating transitions of care and continuity of services as patients needs evolve.
- Implement infection control practices safety measures and emergency protocols to protect patients caregivers and oneself from harm adhering to established guidelines and protocols.
- Participate in interdisciplinary team meetings case conferences and quality improvement initiatives to review patient progress identify barriers to care and enhance care coordination and outcomes.
Qualifications:
- Graduate of an accredited practical/vocational nursing program with a current license to practice as an LPN or LVN in the state of employment.
- Minimum of 23 years of nursing experience preferably in home health community health or a related healthcare setting demonstrating proficiency in skilled nursing procedures and clinical assessments.
- Strong clinical assessment skills critical thinking abilities and proficiency in administering medications treatments and procedures safely and effectively.
- Excellent communication interpersonal and patient education skills with the ability to establish rapport with patients and families from diverse backgrounds and cultures.
- Knowledge of nursing principles practices and standards of care relevant to home health nursing including regulatory requirements and best practices in communitybased care.
- Ability to work autonomously manage time effectively and prioritize tasks in a dynamic and fastpaced home health environment.
- Proficiency in using electronic health record (EHR) systems and other technology platforms for documentation communication and care coordination.
- Valid drivers license reliable transportation and willingness to travel to patients homes within the designated service area.
- CPR certification and other relevant certifications may be required depending on state regulations and organizational policies.
