Position: Case Manager 1
Program: Roger Sherman House
Location: New Haven
Salary: $18.50/hour
Schedule: Full time 40 hours 1st shift TuesdayThursday & Saturday 73p) and 2nd shift Friday 311p)
Program Summary:
Roger Sherman House is a residential work release program that provides individualized treatment services job preparation substance use education and mental health instruction to adults under the supervision of DOC. These services are provided so they can successfully reenter the community while avoiding new offenses.
Position Summary:
The Case Manager 1 is responsible for providing staff support and direct client care. Responsibilities include delivering comprehensive case management services to clients including intake assessments development and implementation of individual care plans and liaison with treatment providers and community resources. This position is nonexempt and works under the supervision of program Management.
Working within Adult Services the Case Manager 1 works with Community Justice program staff to help to rehabilitate educate and successfully guide clients from incarceration back into the community.
Requirements:
- Some postsecondary education
- 2 years of related experience in the direct service and case management field
- Valid Connecticut drivers license
- Regular and predictable attendance is required
- The Connection Inc. complies with Executive Order 13G. Any employee hired on or after September 27 2021 must either be vaccinated against COVID19 or apply for a medical or religious exemption and be approved. If approved you must submit to weekly testing as required in each program.
Orientation:
If selected for employment with The Connection newly hired employees will be required to attend a two 2 day mandatory New Hire Orientation at our Roscommon office located in Middletown.
