CVHCare a leader in Home Health Clinical services headquartered in beautiful San Ramon CA is currently seeking a Physical Therapist to join our Home Health Care Agency located in Woodland Hills CA. Open to entry level or candidates with no prior home health experience
Title: Physical Therapist PT
Schedule:Full Time 24 visits per week) Part Time 18 visits per week) or Per Diem 6 visits per week)
Locations Available:Area #45 covering the cities of Agoura Hills Calabasas Moorpark Newbury Park Oak Park Simi Valley Thousand Oaks Westlake Village and Area #42 covering the cities ofCanoga Park Chatsworth Encino Lake Balboa Reseda Tarzana West Hills Winnetka and Woodland Hills
Paid Training: Must be able to train for 5 days MonFriday 9am5:30pm in Woodland Hills
Position Summary: Initially a PT will evaluate mobility strength and current level of function while setting goals to improve balance walking transfers as well as patient education. Unlike a rehabilitation setting or even an outpatient clinic unique emphasis is placed on safe mobility in and around the patients residence. PTs will also make recommendations on equipment and provide train on proper use of equipment.
Requirements:
- 1 year Physical Therapy experience
- Open to entry level or candidates with no prior home health experience
- Reliable Transportation
- CPR/BLS Certification
- Must be available for one week of orientation and training
