Job Summary:

The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking an hourly/per diem Outpatient Retail Staff Pharmacist. The Staff Pharmacist reports to the Associate Director of Retail Pharmacy Services and prepares compounds and dispenses medications for outpatient and ambulatory care use as well as dispenses retail and specialty pharmacy related items. Additionally the successful candidate will:

  • Provide information to health care practitioners to promote the rational use of medications.
  • Provide drug monitoring service.
  • Provide prescription medication counseling to patients as requested prior to discharge time of dispensing and at patients bedside.
  • Participate in teaching programs for staff.
  • Monitor orders to ensure individualized dosage for both pediatric and adult patients.
  • Monitor serum levels and make individualized patient recommendations based on pharmacokinetic and pharmacodynamic properties.
  • Apply knowledge of pharmacology and pharmacokinetics to prevent duplicate therapy and drug interactions.
  • Provide direct supervision of medication preparation and dispensing; demonstrate an efficient use of resources (labor services supplies/materials communication/data and space).
  • Conduct mediation reconciliation and drug utilization review (DUR) to assure safety and appropriateness of medications.
  • Review patient profile to ensure appropriate drug selection and dosage (no drug duplication interactions or allergies) and route of administration with accurate and complete information.
  • Document all relevant interactions with patient and health care providers; document adverse drug reaction order clarifications per state and federal laws and regulations.
  • Efficiently perform pharmacist duties including the safe procurement preparation labeling distribution and control of all .
  • Participate in the pharmacy oncall rotation as applicable.
  • Complete quality assurance functions including quality improvement projects as assigned; perform all other duties as assigned by management.
  • Contribute to the organizations overall process improvement activities; demonstrate critical thinking skills in identifying and analyzing problems and recommending solutions.
  • Promote a culture of safety as it relates to identifying safety and security risks proactively and working to prevent injuries and/or harm to patients visitors or members of the SUNY Downstate community; observe the patient safety goals and commit to safe behavior and standards.
  • Oversee inventory management of both controlled and noncontrolled substances according to state and federal laws; maintain pharmacy records as required by law and institutional policy.
  • Participate in performance improvement activities; participate in and maintain training and education for competence required for the Staff Pharmacist position; complete all ongoing education and training as assigned by the department.
  • Interact with others in a positive and friendly manner; adhere to patient experience philosophy and customer service standards; represent SUNY Downstate in a positive manner focusing on institutional core values which drive innovation and discovery respect of people service to community; value integrity and embrace diversity and inclusion.
  • Demonstrate sensitivity to patient/family special needs and be accountable for addressing each special need (unique personal history physical limitations belief system language communication style literacy health status etc..
  • Use ethical principles to guide decision and actions; recognize the value of each individual within a diverse and multicultural environment; treats others with respect and dignity.

Required Qualifications:

  • Doctor of Pharmacy (PharmD).
  • New York State licensure and registration.
  • Immunization certified.
  • Ability to exert up to 50 pounds of force occasionally and up to 20 pounds frequently and up to 10 pounds of force constantly in order to move objects.

Preferred Qualifications:

  • 2 to 3 years of retail pharmacy experience.

Commensurate with experience and qualifications
