This position is a full time hybrid position; 40 hours weekly 8hour shifts working Monday Friday 8am4:30pm. No weekends or holidays.

The successful candidate will provide population health management services to patients of the Medical Group of ChristianaCare. The Clinical Pharmacist is responsible for completing comprehensive medication reviews for eligible patients as well as ensuring that performance targets are met or exceeded for medicationrelated measures across valuebased contracts. Collaborates with providers and patients to ensure patients are meeting applicable measures.

Patient Care:

  • Designs and implements clinical programs to enhance population health.
  • Collaborates with patients providers and payerbased pharmacists to optimize outcomes in riskbased contracts with a focus on CMS STARS and HEDIS quality measures.
  • Provides comprehensive medication management services and annual wellness visits to eligible patients.
  • Utilizes risk stratification tools to identify targeted highrisk patient population dependent on age hospitalizations ED visits comorbid diagnoses (e.g. COPD CHF Diabetes) polypharmacy and behavioral health diagnosis.

Quality Improvement:

  • Integrates IT into workflow to enhance communication about medications across care settings.
  • Represents pharmacy on departmental and health system committees as assigned and contribute when pharmacotherapy issues are discussed.
  • Supports and assists interdepartmental projects as requested (e.g. newsletter articles performance improvement activities etc..

Research and Scholarship:

  • Participates in research design data analysis and publication of study results as a preceptor for resident research projects.
  • Contributes to the pharmacy and medical literature (e.g. case reports letters to the editor and therapeutic pharmacokinetic and pharmacoeconomic reports peer reviewer).

Education and Experience Requirements:

  • PharmD from an ACPE accredited School of Pharmacy Required
  • ASHPaccredited PCY 1 residency required
  • ASHPaccredited PCY 2 ambulatory care residency preferred
  • BPS board certification preferred
