Description
Organization Overview:The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are wellestablished Manhattan communitybased agencies that support services to approximately 30000 New Yorkers. We are two of New York Citys leading human services organizations meeting the needs of children youth and families homeless individuals and older adults throughout Manhattan and New York City. We work every day for a fair and just society where all people have the opportunity to make choices that lead to better lives for themselves and their families. The Isaacs Center focuses primarily on the needs of children and lowincome families outofschool and outofwork youth and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville and seek to deliver programs that are impactful innovative and intergenerational.
Program Description: Goddard Riversides Corner House provides affordable permanent supportive housing for 34 previously homeless adults many of whom have mental illness substance use and other cooccurring disorders. Onsite services include case management entitlement assistance medical and psychiatric care substance use counseling group activities recreation and 24 hour/7 day per week front desk security.
Position Summary: To deliver effective case management services using a strengthsbased clientcentered approach to formerly homeless adults diagnosed with a severe and persistent mental illness in order to maintain permanent housing. The Senior Case Manager will provide administrative support while carrying a caseload. The Senior Case Manager will supervise guide oversee employee performance and take control of implementing improvements on a daily basis. As a Senior Case Manager you must demonstrate Integrity flexibility problemsolving direct moving projects along faster and improve efficiency. Senior Case Manager must demonstrate strong decisionmaking skills and clear communication.
Schedule: MondayFriday 9:00am5:00pm
Salary Range: $53000$60000
Candidates must have the required qualifications to be considered for the position. The salaryoffer will be basedonthe candidates number of years experience in the exact orcomparableposition and additional preferred qualifications will be taken into account.

1 Client service
- Provide case management to a group of assigned clients; identify short and longterm treatment goals and help clients achieve them including linkages to community programs such as medical psychiatric and drug treatment services.
Assist clients in receiving and maintaining entitlements
Assist clients in understanding and complying with building rules to maintain residency
Maintain case records with documentation of services provided and outcomes
Responsible for case notes and service plans using an electronic database
Coordinate meetings with clients and building management
Coordinate and track medical appointments between tenants and various medical providers
- Assist in taking clients on outdoor events when low on staff
2 Administrative Duties
- Assist the Program Director with training new social service staff
- Complete case record audits monthly for the Program
- Participate in tenant intake/admission meetings
- Support the Program Director in serving as a Liaison to other community services; attend meetings and develop new linkages in the absence of the PD
- Serve as senior staff member in Directors absence
- Track and maintain records for staff trainings and schedule staff needed.
- Coordination of programs and community referrals
- Provides culturally responsive crisisintervention advocacy and supportive counseling services to clients and supervises the case management team.
- Participates in ongoing training and educational opportunities; provides educational resources to staff and volunteer advocacy
- Develops ongoing referral relationships with partner organizations and within the health legal and social services systems
3 Participate as Corner House team member
- Attend staff meetings case conferences training programs and supervision meetings
- Attend Incident Review Committee meetings in the absence of the Program Director
- Collaborate to maintain an active resource directory
- Conduct apartment inspections in conjunction with building management
4 Other duties as required in accordance with agency polices and funding source requirements
Qualifications/Educational Requirements
- Bachelors Degree required in related field. Masters Degree preferred
- 2 to 3 years of experience in a Social Services setting
- Experience working with individuals diagnosed with severe and persistent mental illness substance abuse and/or cooccurring disorder
- 23 years Case Management experience with some supervisory experience
Skills Knowledge and Abilities
- Excellent organizational oral and written communication skills
- Spanish fluency is a plus
Computer Skills: To perform this job successfully an individual should be:
- Proficient in Microsoft Outlook and Excel
- Able to use or learn computer databases such as AWARDS TMS
Physical Requirements
- Some walking and travel to escort clients to various appointments throughout the city
- Occasional visits to hospital emergency rooms Nursing home
- Walking upstairs
Work Environment
- Mostly indoor office occasionally in kitchen during groups
- Regular visits to client apartments also required
