The Clinical Fellow Speech Language Pathologist provides assessment and treatment of patients with speech language voice swallowing social pragmatics fluency and cognitive disorders. Duties related to patient care include administration and interpretation of formalized and informal assessment and procedurebased assessment. The Clinical Fellow Speech Language Pathologist plans and develops treatment programs for patients often in conjunction with other medical professionals in a teambased setting. The clinician provides education of the patient family members and staff documents assessment findings recommends treatment plans reports out to other members of the patient care team and provides clinical treatment. Patient/client assessment consultation and treatment are conducted using broad guidelines and require a high degree of independence and innovation in decision making. Clinical supervision is provided.
The Clinical Fellow manages technical environmental and financial resources effectively.
Problems encountered can generally be solved by referring to program resources subject matter experts and or practice leadership. Supervision will be provided based on the American Speech Hearing Associations (ASHAs) requirements patient presentation and employees clinical progression. Guidance from the clinical supervisor is provided regularly and consultation with other staff occurs regularly. The Clinical Fellow must problem solve how to meet patient care needs and professional standards regarding documentations of patient care.
The Clinical Fellow Speech Language Pathologist is a mentored professional experience that begins after the completion of academic curse work and clinical practicum. The goal is to transition between being a student and being an independent provider of speechlanguage pathology clinical services.
The purpose of the Clinical Fellow Speech Language Pathologist is to integrate and apply knowledge from academic education and clinical training evaluate strengths and identify limitations develop and refine clinical skills consistent with the Scope of Practice in SpeechLanguage Pathology and advance the Clinical Fellow from needing consistent supervision to being an independent practitioner.
As a requirement of the position the Clinical fellow has regular access to and knowledge of confidential medial history of patients/clients in their care. Documentation is required and is in accordance with professional and regulatory guidelines.
DURATION OF THE CF EXPERIENCE
As a Clinical Fellow you must work at least 36 weeks and 1260 hours to meet the ASHA CF experience requirement.Within this time frame a clinical fellow must have 18 hours of direct and 18 hours of indirect supervision during the full clinical fellowship experience.Clinical Fellow experience should be divided into 3 segments 12 weeks) with each segment representing onethird of the total. Each third should include observations and supervisory activities required by the Clinical Fellow mentor. At least 3 observations hours per segment and per mentor must occur in person.
A minimum of 80 of the Clinical Fellow experience must be spent in direct clinical contact related to the management of disorders that fit within the ASHA Scope of Practice in SpeechLanguage Pathology. Examples of direct clinical contact include the following:
Screening Response to intervention and/or observation of the client/patient
Assessment/Diagnostic evaluations
Treatment
Writing of reports notes progress notes: billing
Family client consultation
Family counseling
Up to 20 of the Clinical Fellowship may be spent doing other activities such as attending inservices or providing training and presentations.
To provide an accurate rating of a Clinical Fellows Independence with each skill on the Clinical Fellowship Skills Inventory (CFSI) each clinical mentor must complete a minimum of 6 hours of onsite and in person direct and 6 hours of indirect observation during each segment that you serve as their Clinical Fellow ship mentor. This equals to 18 hours of direct and 18 hours of indirect supervision during the full clinical fellowship experience. You may provide more supervision if your local and or state policies require more or if you feel that your Clinical Fellow would benefit from more supervision.
During each segment of the Clinical Fellowship experience each Clinical Fellow mentor must provide at least 6 hours of direct observation of Clinical Fellow as they provide evaluation or treatment services to patients/clients. Remember a segment is equal to one third of the clinical fellow experience. At least 3 observations hours per segment and per mentor must occur in person.
Each mentor must also provide per segment 6 hours of indirect observation which typically include reviewing diagnostic reports/treatment records plan of treatment monitoring the Clinical Fellow in case conferences or professional meetings or evaluation the Clinical Fellows work by consulting colleagues or clients and families. The Clinical Fellow mentor and the Clinical Fellow must coordinate the observation schedule to ensure that all the skills are observed and evaluated during each segment of the Clinical Fellow experience. It is expected that the observations are spread over the duration of the Clinical Fellow experience with no more than 6 total hours of observation in 1 day.
Feedback sessions are an integral part of the CF experience. During the feedback sessions the Clinical Fellowship mentor and the Clinical Fellow should discuss strengths and weaknesses in the clinical Fellows ability to independently practice as a clinician. The Clinical Fellow must receive a score of 2 or better on all skills on the 2020 CFSI Clinical Fellow Skills Inventory during the final segment of the CF experience.

The first week of work the CF fellow will complete orientation specific to Trinity Healthcare including general orientation and orientation to Cerner (electronic medical records documentation).
A schedule of evaluation and treatment will follow the Speech Language Pathologist Clinical Fellow such as: The CF will observe 12 Evaluations and 12 treatments from the Clinical Mentor. Then the clinical mentor will observe the Speech Language Pathologist Clinical Fellow for 12 Evaluations and 12 treatments. These will be built into the Speech language Pathologist Clinical Fellow schedule the first week of scheduled work.
After successful evaluations and treatments are completed based on observation by the Clinical Mentor the Clinical Fellow will have a limited schedule assigned to them the first week of employment. This schedule is based upon the following: assessment evaluation documentation treatment plan goals objectives and patient/client/family/caregiver interaction. Should the Clinical Fellow or Clinical Mentor feel that more observation or teaching is necessary this can be built into the Clinical Fellows first week of work to help ensure a successful clinical fellowship year.
MINIMUM QUALIFICATIONS AND REQUIREMENTS
Licenses and Certifications Required
North Dakota License to Practice as a Speech Language Pathologist.
Basic Life Support (BLS/CPR) required within 30 days of hire.
Educational Requirements
Masters degree in Speech Language Pathology.
Experience Requirements
In the process of obtaining a Certificate of Clinical Competence in Speech Language Pathology.
Preferred Practicum (student) experience in a medical or outpatient setting.
Special Skills or Training Requirements
Excellent communication and interpersonal skills
Excellent analytical skills
Occasional work may be required on weekends and holidays.
