Job Description
Located in the Chestnut Hill section of Philadelphia Chestnut Hill Hospital is a 148bed communitybased universityaffiliated teaching hospital committed to excellent patientcentered care. Chestnut Hill Hospital provides a full range of inpatientand outpatient diagnostic and treatment services for people in northwest Philadelphia and eastern Montgomery County. With more than300 boardcertified physicians Chestnut Hill Hospitals specialties include minimally invasive laparoscopic and robotic surgery cardiology gynecology oncology orthopedics urology family practice and internal medicine. Chestnut Hill Hospital is accredited by The Joint Commission and is affiliated with universityhospitals in Philadelphia for heart and stroke care and residency programs.
Temple Health plans to acquire Chestnut Hill Hospital as part of an alliance with Redeemer Health and Philadelphia College of Osteopathic Medicine. The newlynamed Temple HealthChestnut Hill Hospital will be managed by Temple Health System and the transaction is anticipated to be completed by the end of 2022.

The Case Manager is a registered nurse responsible for coordinating the care and services for an assigned unitbased patient population. The Case Manager promotes quality of care supports effective resource utilization and facilitates the patients progression of care to proactively plan for the patients safe and timely discharge to the most appropriate level of care. The Case Manager serves as the key resource for the patients multidisciplinary care team and works collaboratively with all members of the patients health care team and patient representatives to achieve desired clinical financial and quality outcomes. The Case Manager analyzes current systems and variances to prevent delays and identify opportunities for improvement. This position reports to the Manager Care Manager with oversight by the Director. Case Management is collaborative process which assesses collaborates plans implements coordinates monitors and evaluates options and services to meet an individuals health needs through communications and available resources to promote quality costeffective outcomes. Case Management activates its processes to secure the right service to the right patient at the right time at the right level of care.Education Requirements Graduate of an accredited school of Nursing Experience No experience Certification and Licensure PA Registered Nurse License
