Behavioral Health Registered Nurse $5K SignOn Bonus!
Coverage Area: Fall River/New Bedford
Schedule: FullTime Afternoon/Evenings

As a Behavioral Health RN Case Manager youll take the lead in:
Assessing patient needsphysical psychosocial and environmentalto develop individualized care plans
Managing a caseload coordinating and evaluating treatment plans
Conducting initial evaluations and ongoing assessments during patient visits
Communicating changes in patient conditions to the Clinical Supervisor physicians and care teams
Educating patients caregivers and staff on behavioral health philosophy and symptom management
Providing leadership and oversight to LPNs
Offering compassionate support to patients and families
Current unrestricted RN license in the state
1 years of nursing experience in a clinical care setting
Willingness to travel within the assigned coverage area 50
Reliable transportation valid drivers license & auto insurance
Associate or Bachelors Degree in Nursing (preferred)
Experience in home care (preferred)
