Population Health Pharmacist

The PartTime Population Health Pharmacist will serve in a consultative capacity bringing pharmacy expertise to Medical Affairs and clinical teams addressing medication barriers and pharmacy access for CBH youth and adult members. They will also implement and evaluate a framework for more productive clinical outcomes for CBH members in partnership with CBH physician advisors and prescribers in CBH network. This pharmacist will also support team NCQA activities pertaining to the development and evaluation of interventions to improve CBH performance across medication based HEDIS & related metrics.

Salary:

• Minimum hourly rate starts at $57/hour based on experience.

Essential Functions:

• Assist in transitions of care through various levels of care by developing and operationalizing sustainable Reconciliation processes across clinical teams.
• Participate in the monitoring and implementation individuallevel and population health level interventions using standard HEDIS measurements and other nonHEDIS performance metrics that enable CBH continually meets the Program Standards and Requirements as well as remain accredited by National committee for Quality Assurance
• They will also implement and evaluate a framework for more productive clinical outcomes for CBH members in partnership with CBH physician advisors and prescribers in CBH network.
• As requested provide education to members and their caregivers on their medications and the impact their medications have on treatment.
• Provide education as warranted to other health care team members such as Certified Peer Specialists Clinical Care Managers Case Managers Nurses and prescribers.
• Serve as a liaison to PHMCOs and prescribers pertaining to medications and follow up (such as labs) to ensure optimal treatment and resolve of drugrelated problems.
• Contributes to program improvement by using continuous quality improvement processes being pro active in identifying improvement opportunities and by contributing ongoing consultation and collaboration internally within CBH and with external stakeholders.
• Serve as preceptor to pharmacy and public health trainees.

Position Requirements:

• Education: Doctor of Pharmacy Degree (Pharm.D.
• License: ActivePennsylvania Pharmacist License
• Experience length: At least 5 years of pharmacist experience or 2 years with postgraduate training.
• Relevant Work Experience: CBH seeks to engage a pharmacist with experience working in a healthcare setting serving individuals with behavioral health (mental health and substance use) disorders. The ideal candidate will have had experience interfacing with or working in a managed care or public health environment.
• Knowledge of current research related to efficacy adverse effects and outcomes of administration of behavioral health medications and strong knowledge of the prevalence of physical health disorders in individuals with behavioral health disorders across the life span.
• Extra consideration will be given to a candidate with PostGraduate Training or Credentials in Behavioral Health or Ambulatory care
