Job Description

Position Overview: We are seeking a highly skilled and compassionate Social Worker to join our team. The ideal candidate will have extensive experience working with adults dealing with substance abuse homelessness and behavioral health issues. This role is pivotal in supporting our housing with supportive services program which aims to provide stable housing coupled with essential support services to promote longterm independence recovery and wellbeing.

Location: The Apartments at St. Barts 5364 Jackson Street Philadelphia PA 19124
Employment Type: Fulltime

Key Responsibilities:

  • Conduct comprehensive assessments to determine clients needs and develop individualized care plans.
  • Provide counseling and support services to clients dealing with substance abuse homelessness and behavioral health challenges.
  • Collaborate with a multidisciplinary team to coordinate and deliver comprehensive care.
  • Facilitate access to community resources including housing medical care employment services educational opportunities and financial management.
  • Develop and implement evidencebased interventions to support clients recovery and stability.
  • Maintain detailed and accurate records of client progress and services provided.
  • Advocate for clients rights and needs within the community and with service providers.
  • Participate in regular training and professional development to stay current with best practices in social work and supportive services.

Qualifications:

  • Bachelors degree in social work from an accredited institution preferred.
  • Minimum of 3 years of experience working with adults in substance abuse homelessness and behavioral health settings.
  • Excellent assessment counseling and case management skills.
  • Ability to work effectively in a teamoriented collaborative environment.
  • Exceptional communication and interpersonal skills.
  • Commitment to diversity equity and inclusion in service delivery.
